What happens at the moment of confirmation

No balance is held anywhere in the middle. The cashier composes a payment instruction, hands the customer to their own bank where it is authorised with the usual credentials, and receives a message confirming the payment was initiated. Crediting happens on that message, not on the eventual clearing — which is exactly why the deposit is immediate even though the underlying money moves later.
